June 8, 2006 Submit workers' comp forms online starting Oct. 1
The First Report of Injury (FROI) for workplace injuries and the Lost/Restricted Time Weekly Report for employees unable to work due to a workplace injury may now be completed online. Beginning July 1, all colleges and departments will be encouraged to submit the two forms through the online process. Beginning Oct. 1, the forms must be submitted online. To complete the online process, employees or their supervisors will log into AccessPlus, go to the "Employee" tab, select "Work Injury" and "Continue" to view the FROI homepage. Employees or their supervisors can submit a FROI and Lost/Restricted Time Weekly Report, as well as view previously submitted weekly reports. The online version is intended to simplify the process and save time. Employees or their supervisors only have to fill in a few required fields and review the report. Employees will submit the report to their supervisors. Supervisors who receive the report or complete it for their employees will submit the report to the proper person in their units or Human Resource Services. Not all forms concerning workers' compensation may be completed online. Employees must fill out the P-28 form (workers' compensation benefit election) since it requires a signature. It is online at http://www.hrs.iastate.edu/workerscomp. The signed form should be turned in to the Office of Workers' Compensation, 3680 Beardshear, or faxed to 4-7530.
